It would take too long to just write the entire story down first. They usually come up with an idea and try to get in shonen jump or one of the others like it
Not only that they go on 20 hiatuses during the run insuring the anime/manga you started as a child ends when your an adult then you notice if you started that shit when you were older you would have no hope of finishing it in a appropriate amount of time. Unless the author dies before it ends like berserk
